Versorgungsforschung: Retrospektive Datenanalyse von Krankenhausakten zur Über- oder Unterdosierung von Protonenpumpenhemmern bei stationären Patienten eines süddeutschen Akutkrankenhauses

Proton pump inhibitors (PPIs) are among the most commonly prescribed medications worldwide. While the literature primarily discusses overprescription, this retrospective study paints a more nuanced picture.

An analysis of over 2,000 patient cases at a hospital in southern Germany from 2018, 2020, and 2022 revealed that 88% of patients were treated in accordance with guidelines upon admission, and 83% upon discharge. Undertreatment at discharge proved particularly problematic—especially among cardiology patients with acute coronary syndrome for whom dual antiplatelet therapy or a combination of antiplatelet therapy and oral anticoagulation was newly initiated.

Furthermore, the retroactive application of the updated 2022 S2k guideline “H. pylori and Gastroduodenal Ulcer Disease” highlights a significant need for action: Undertreatment would rise to 27% because key recommendations (ASA, P2Y12 inhibitor, DOAC, or VKA in combination with a risk factor) have been tightened. Thus, the study highlights the risk of undertreatment with a widely used medication, particularly in relation to the transition to the updated 2022 S2k guideline.
